In my case, I got 12 months of Xbox Live for 41 bucks, no tax, no shipping. I ordered around 10-11 EST on a Saturday, and got it literally at 12 AM Sunday. Not quite "immediate," if you're in a tight spot, but pretty freakin' good if you don't feel like spending extra and don't mind waiting at the most a day, two tops, or most likely just a few hours.

It seems sketchy, but it works. Can't speak for CD-keys of *actual* games, but for one-time prepaid codes such as in-game currency and subscription extensions, this site is definitely the place to go.
