“CDW double charge”

Booked through the UK site. CDW coverage is not an option, it can't be unticked. The terms and conditions document says CDW is included. The rental confirmation does not, in my case. The US National desk at Ft. Myers airport RSW insisted that CDW was not included. They charged an extra 175 dollars. National(Enterprise) help desk are no help at all. Now, is Nationalcar.co.uk wrong, or is National RSW wrong?
