I am an elderly and not very frequent traveler. I booked a flight from Heathrow to Mexico which included a change of plane at Dallas Airport. Now I am aware that a visa is required when entering the USA but was not aware that one was required when simply changing planes without leaving the airport. Consequently on arrival at Heathrow I endured a nightmare hour attempting with my minimal computer skills to apply on line for a temporary visa during which I paid $65 to what turned out to be the wrong site, then a further $14 after I had declared my intention of going home and a sympathetic booking clerk reapplied for me. In fairness, buried in the Netflights small print is the recommendation that one checks the necessity of securing a visa BUT this necessity did not even occur to me. I know that had I booked my flight via a travel agent I would have been reminded that a visa was necessary and in future that is the route I shall follow
