Hi Jim,
sorry fella i can only say to you that the service provider is the only person who can give you the PUK code, trust me i have been to a few hack sites and i get told every time that the code is to hard to generate and must be provided by your service provider.
If you registered the sim( in your name ) you can ring your service provider and ask for help, which they will give you if it is registered in your name.
However if you do not have the sim card registered in your name you can ask them to de-register the sim which can take about 30 days as they send a letter out to the previous owner( if this is applicable ie your sim card is pre-owned/second hand ) and then they let you know if you can re-register the card.
The above paragraph is not applicable to & quot;orange & quot; as they are stricter with there data and do not want the hassle of registering and re-registering sim cards. I know this as you can see from the original post i had an orange sim and had to get another sim as they would/could not help me with my PUK code. Very frustrating as my whole phone book was on the sim. Remember to write your phone book to the phone memory and even if your sim fails your phone will retain the info. I know its good advice that is late but i have just learnt the hard way=(
Hope this helps Jim.
