A puk needed is a function of the simcard, not the phone, you will need to contact customer services of t-mobile, http://www.t-mobile.co.uk/Dispatcher?menuid=help_cu_cs , here is the blurb from their website: If your smartcard is blocked (by entering the wrong PIN number), you will need to enter your Personal Unblocking Key. This is supplied with the smartcard, and can be obtained by calling Customer Services. If you have lost your PUK, please contact them immediately - if you enter the wrong PUK, you could lock your handset indefinitely.
