“Dishonestly charging for extras declined verbally”

Rented a car at Boston Logan on 18th June. This is a trip I make regularly, pre-booking a car in Europe. I always make it clear to the agent that I do not require any extra, or extra insurance. This time the agent (who was actually a manager) went through the normal routine, and I again made it clear that I didn't want any extras. He then gave me the electronic form to accept/decline, and a few weeks later I find that I accepted some extra charges. He must have known he was doing this, and that I might not notice. Well, he succeeded in deceiving me, but if this is how customer service at Avis works, I go elsewhere in future.
