I travel a lot over Europe and US for business and holiday trips using various car rental companies services, but booking only at economybookings.com for already about two years. Moreover, you don't have to worry about the insurance, because it is included, and mileage, because it is usually unlimited.
The condition of cars depends on a supplier but, based on my experience, in most cases they were almost new, with a low mileage and clean. There are another possibilities that terms and conditions include, for example, sometimes you have to return the car with a full tank and sometimes not, and I prefer the second option.
