I had a seven seater booked with Avis through BA in Orlando and did not require an upgrade, as it was already several upgrades from the basic.
After a long flight we were persuaded into an extra $5 a day for an upgrade we did not request and I questioned the deck clerk who said we would get it back. I also phoned the company from my hotel who also said this was a charge I would get back. When I received my bill when I got home, of course I was charged. I have never upgraded in the USA if I wanted to I would do it before I fly.
